The onesies are better for not bunching up under a sleeper but a snap side shirt can be fine . Congrats and good luck to you.

If you are talking about a sleepsack, you need a onesie. The shirts bunch up in my experience. If you are talking about a footed sleeper, I cant see putting something on under it at all.
